I am an out-of-network provider with all insurance plans and you will be responsible for paying me directly for the full costs billed for services rendered. I am happy to provide you with invoices that you can submit to your carrier and/or Flex Spending or Health Savings Account (FSA/HSA) to be reimbursed directly per the terms of your plans. I accept payment via check and also Visa and MasterCard (including FSA cards). Please contact me for a fee schedule.
I encourage you to contact your insurance carrier to understand the details of any out of network benefits, including rules related deductibles, co-insurance, allowed amounts, and limits. Fees paid to an out of network provider might not count toward your deductible or out of pocket limit. You can choose to get care from a provider or facility in your health plan’s network, which may cost you less.
You have the right to request and receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Depending on patient needs, I sometimes recommend a referral to additional healthcare providers. When requested, as a courtesy I am sometimes able to provide a list of recommended providers, though you are welcome to choose any provider whether or not on this list. I have no knowledge of other providers' billing practices. You will need to contact the provider to obtain further information, including whether that provider is in-network with your insurance plan. Your health insurance plan should also have a toll-free telephone number and website where you can find out what providers are in your network and receive estimates and possible charge limits for specific services.
